The Process

Our 7-Step Homecare Process was designed with you and your loved ones in mind. Our individualized plans combined with the top caregivers in Jefferson and Shelby counties has provided our clients with care that they can trust.

Step 1: Initial Call

On your initial call, you will speak to a knowledgeable staff member who will be your main point of contact throughout the intake process. After listening to you and collecting some pertinent information, an appointment will be made for a FREE, no-obligation home assessment.

Step 2: In-Home Assessment

Our Client Care Coordinator will visit you to assess your needs, home care issues, and the qualities of a caregiver you seek. The assessment includes:

  • Medical and Medication profile

  • personality profile

  • nutrition analysis

  • home safety inspection

*The assessment requires no obligation on your part. You will not be asked to sign anything at that time unless you wish to proceed with services.

Step 3: Plan of Care

Our Client Care Coordinator will create an individualized Plan of Care to be implemented by your home care team. This mutually agreed-upon Plan of Care is created in conjunction with the client’s and/or family’s recommendations. A copy of the Plan of Care can be forwarded to the client’s Primary Care Physician.

Step 4: Caregiver Match

Our office staff spends a great deal of time and effort evaluating your home care situation and personality requests, and then carefully selects a caregiver(s) we believe, from our experience, would be the “Right Fit”. For example, some people prefer someone who is talkative, while someone else might like someone a little quieter. Another example is that some people prefer someone who takes direction well, while others require someone more assertive. If the match is not correct the first time, we will find another caregiver for you. We will not rest until we find the “Right Fit

Step 5: Caregiver-Client Introduction

To ensure a comfortable transition, we prioritize the client-caregiver introduction. During this period, our team equips your new caregiver with the tools to assist in establishing a successful relationship. We provide them with your:

  1. Plan of Care

  2. Environment Characteristics

  3. Your preferred social dynamic

 

The first few days/weeks are crucial to the relationship’s long-term success. This is not a common practice, and we believe is indicative of passion we have for our work.

Step 6: Ongoing Supervision

Our Client Care Coordinator conducts a supervisory home visit every 2-3 weeks to ensure the Plan of Care is being properly followed or needs any revisions. This is another major differentiator for our home care service.

Step 7: Ongoing Support

Each caregiver is managed by our client services team which includes our scheduler and a client care coordinator. Our client services team is responsible for the day-to-day scheduling and care issues and serves as your dedicated liaison with our agency. It is their job to personally know you and your family.
